Bank Letter

If the signor(s) names(s) are not printed on your checks, or the bank account is for a business, organization, or other entity, a letter from your bank is needed.

The letter from the bank should be on bank letterhead, be signed and dated by an authorized bank representative and contain the following:

• Bank Account Title
• All bank account signors' names
• Account routing number
• Account Number
• Account type (checking or savings)
